MAKING AN APPOINTMENT
All Patients are seen by appointment only.  If you are a new patient or if you have changes to your account, please arrive 30 minutes ahead of time to complete necessary paperwork.

REFERRALS/HOSPITALIZATION
If a provider refers you to a specialist, it is your responsibility to make sure the referral is pre-authorized.  We do provide the services of a Patient Coordinator if you have questions regarding your insurance authorization, referrals or follow-up appointments.  Please call 270-885-2091.

PRESCRIPTIONS REFILLS
Please call your pharmacist at least 2 days prior to needing your medication.  Your pharmacist will call our prescription line and leave a message.  If you need to reach our dedicated voice mail prescription line, 24 hours a day, call 270-886-6891 or fax your request to 270-885-2094.  Saturday and holiday refills cannot be provided.

LABORATORY
Our lab is an OSHA and CLIA certified moderate complexity lab equipped to do numerous tests.  If any test results are abnormal you will be notified by phone by a member of our clinical staff, otherwise you will receive notification of your lab result by mail.
